Scott Reserve Request Form

Copies of copyrighted materials must be accompanied by a Library Course Reserve Submission Form. The form must be signed by the instructor. Items may be delivered in person to the Scott Library Circulation / Reserve desk or mailed to Scott Circulation Services, 133C Scott Library.
